Schedule: Monday-Friday, 9:00am-6:00pm
Clinics from 9:00am-5:00pm

To enroll: Download this form, fill it out and bring it, along with your payment, to the office of the Danilo Perez Foundation, located in the Old National Conservatory, in front of Plaza Herrera, in Casco Viejo.  If you are unable to download the form, you must come to the office in order to fill out the form and pay the enrollment fee.

NOTE: In order to take advantage of the pre-enrollment price, you must pay before January 8, 2010.
